Waivers for Private Lessons of 10 or less people

If 10 or less people are confirmed for a private lesson, then individual waivers will be required before the lesson begins(QR codes will be available at the front entrance) or they can be located here or below if the website is down for maintenance

Waivers are only required for

• NEW 1st Time Participants
• RETURNING Participants who have not come in for a lesson in the last 365 days or more. A new waiver is required in this instance 

Returning students who have been back in less than 365 days will not be required to fill out a new waiver
